Insurance Intermediary Information
This information is provided to the client before the conclusion of an insurance contract, pursuant to part 1 of Article 88 of the Law of Ukraine “On Insurance”.
LIMITED LIABILITY COMPANY “Ensuria Operations Ukraine” (hereinafter — the insurance intermediary, subagent), within the scope of providing insurance services, acts as an insurance intermediary of the Insurance agent LLC “Polis.UA” and, pursuant to the Law of Ukraine “On Insurance”, provides the following information:

Insurance intermediary: Subagent.
LLC “Ensuria Operations Ukraine”, acting under Agency agreement No. 07072026/1519 dated 07 July 2026, is authorised, on behalf and on the instructions of the Insurance agent LLC “Polis.UA”, to carry out part of the insurance activity, namely:
— advertising and/or carrying out marketing, promotional and other preparatory activities aimed at the conclusion of insurance contracts, and providing information on the terms of insurance contracts;
— providing expert, informational and consulting services in the field of insurance;
— independently searching for potential consumers of insurance services;
— consulting potential consumers of insurance services and providing them with proposals for the conclusion of insurance contracts (including in electronic form via the information and telecommunication system) and the performance of insurance contracts;
— carrying out activities related to the preparation for the conclusion of insurance contracts.
The sale of insurance products and the conclusion of electronic insurance contracts are carried out by the Subagent via the information and telecommunication system of the Insurance agent.

7. Type of remuneration for the conclusion of an insurance contract, and the procedure and terms of its payment
For the conclusion of an insurance contract / facilitating its conclusion, the Insurance intermediary (subagent) receives a monthly commission, based on a certificate of services rendered, as a percentage of the insurance premium under the insurance contract (the remuneration is included in the insurance premium).
8. Amount and method of payment for the services of the Insurance intermediary (subagent) directly by the client
The client does not pay for the services of the Insurance intermediary (subagent).
9. Any other payments (other than the insurance premium) that the client is obliged to make under the insurance contract after its conclusion
The client is not required to make any payments (other than the insurance premium) under the terms of the insurance contract after its conclusion.
10. Mechanisms and means of protecting the rights of financial services consumers (including the possibility and procedure for out-of-court consideration of consumer complaints, and the insurer's address for complaints)
Complaints from financial services consumers are considered and resolved by the Insurer within one month of receipt; complaints that do not require additional examination — within fifteen days of receipt. If the issues raised cannot be resolved within one month, an authorised person of the Insurer sets the necessary review period, which may not exceed forty-five days.
A complaint may be made verbally by phone via the contact centre of the Insurer and/or the insurance agent, or in writing to the Insurer's e-mail or by post to its address. A complaint against the Insurer's actions or decisions may be filed with:

Filing a complaint with the Insurer or a competent authority does not deprive the consumer of the right to apply to a court in accordance with applicable law, and, in case of disagreement with the decision on the complaint, to apply directly to a court.

class 1 — accident insurance (including occupational injury and occupational disease);
class 2 — sickness insurance (including medical insurance);
class 3 — insurance of land vehicles (other than railway rolling stock);
class 7 — insurance of goods in transit (including cargo, baggage (cargo-baggage));
class 8 — insurance of property against fire and dangerous natural events;
class 9 — insurance of property against damage caused by hail, frost and other events (including theft, robbery and intentional damage/destruction of property), other than the events defined in class 8;
class 10 — liability insurance arising out of the use of a land vehicle (including carrier's liability);
class 13 — other liability insurance (other than defined in classes 10, 11, 12);
class 16 — insurance of other financial risks (other than defined in classes 14, 15);
class 18 — insurance of costs related to the provision of assistance to persons who get into difficulty while travelling;
class 19 — life insurance (other than provided for in classes 20, 21, 22, 23).
12. Methods of paying the insurance premium under insurance contracts to the current account of the insurance intermediary (subagent)
The insurance premium is not paid to the account of the Insurance intermediary (subagent).
13. Methods of making insurance payouts by the insurance intermediary (subagent)
The Insurance intermediary (subagent) does not make insurance payouts.
14. List and procedure for the provision of assistance services by the insurance intermediary (subagent)
The Insurance intermediary (subagent) provides assistance services to persons who get into difficulty while travelling.
15. Procedure for processing consumers' personal data and the actions of the insurance intermediary (subagent) to protect it
The Insurance intermediary (subagent) complies with the Law of Ukraine “On Personal Data Protection” and the Insurers' internal regulations on the processing and protection of financial services consumers' personal data.
